ABSTRACT

This prospective, randomized study compared the effectiveness of crystalloid preload

and crystalloid coload in the prevention of spinal anaesthesia-induced hypotension

during Caesarean section.

Eighty paturients with American Society of Anesthesiologists’ (ASA) physical status

1 or 2, with full term singleton pregnancy and scheduled to undergo lower segment

Caesarean section under spinal anaesthesia were randomly assigned into two

treatment groups. Group A received crystalloid preload of 20 ml/kg while group B

received a similar fluid load as coload. Patients’ haemodynamic changes, complaints

of nausea, episodes of retching/vomiting and vasopressor use, as well as neonatal

Apgar score, were recorded during the period of the procedure.

The incidence of hypotension in the preload and coload group in the pre-delivery

period were 35.0% and 26.3% (p = 0.690) while incidence of hypotension for the

entire procedure were 45.0% and 39.5% (p = 0.244) respectively. In the preload

group, 25% had complaints of nausea against 18.4% in the coload group (p = 0.495).

The incidence of retching/vomiting in the parturients and neonatal Apgar scores were

similar in both groups.

There was no difference in the incidence of spinal induced hypotension and spinal

induced hypotension related complications in the parturients following preloading or

co-loading.
